FORMS
All the forms require
Microsoft Word version 6.0 (or higher). All forms are word templates which
allow you to type in your information through your computer or print and
complete by hand. Be careful when filling out any PC version forms
as the formatting may change. You probably already have the Acrobat Reader
software needed to read these files, but if you don't, download it now.
There are some forms listed in the manual that you must get from the Graduate
School (anything with fees, for example). All forms are also available
from the ELPS Graduate Secretary, Kathy Tickle (225 War Memorial Gym)
or your program area secretary.

General
Forms
Change
of Advisor/Committee Request
Complete this form whenever you change your advisor, or add/drop members
to your committees.
Change
of Graduate Program/Curriculum Request
Complete this form if you are changing your area of study from HESA to
another VT Program.
Field
Studies in Education Contract (EDAE 7704) Request
Form should be completed and turned
into the program office prior to the start of classes the semester you
are doing the field study.
Graduate
Student Change Request
If you change your name (as a result of marriage/divorce, etc.), change
your student ID number or social security number, or your student status
from non-degree to degree, this form can do it. Other personal data changes
can be made online through Hokie SPA. This form may require documentation.
Read the form carefully, and contact the Graduate School for more information.
Independent Study (EDAE 5974) Request
You can not register for this class from Hokie
SPA. Once you turn in the form the Graduate School will register you.
Form should be completed and turned into the program office prior to the
start of classes the semester you are doing the independent study.
Internship
in Education Contract (EDAE 5614/7714)
Form should be completed and turned into the program
office prior to the start of classes the semester you are doing the internship.
Leave
of Absence Request
This is a general VT form, it should be completed if you are planning
to take a semester, or more, off from your studies (summer breaks are
not counted as time off from studies). See the your advisor and the Graduate
Catalogue for more information.
Program
Change(s) Request
If you change the order of courses (for example, you were going to take
a course in the spring and have decided to take it over the summer), or
change the courses in your plan of study (Ph.D.) or program of study (MA)
you will need to document theses changes (before you apply for graduation
or take the prelims or prespectus exams), using this form.
Readmission Request
This form should be completed if you are returning from a Leave of Absence,
prior to the start of the semester you are beginning.
Revalidation (Course) Request
If there has been an extended period of time between the completion of
your coursework and the completion of your thesis/dissertation, you may
need to have your coursework evaluated and revalidated. Please see your
advisor for more information.
Reduced Tuition (K-12) Request
Students who areemployees in K-12 while completing their coursework may
be eligible for reduced tuition. Please contact the Grad School for more
information.

for Masters Students
Defending
Student Status
Complete this form if you have taken all course work or program and you
do not need additional hours. It should accompany the Masters Final Examination
Request and the Thesis Final Examination Information form, and be submitted
three weeks prior to your Final Exam.
Masters
Final Examination Request
This form is used to schedule your Final Examination. It should accompany
the Thesis Final Examination Information form, and be submitted three
weeks before the date of your Final Exam. This form is for both the Thesis
and Independent Study options
Program
of Study (MA-HESA)
Before completing 15 credit hours in the program, work with your advisor
to develop your program of study. Any changes to your program of study
need to be documented using the Program Changes Request form.
Thesis
Final Examination Information
This form is for your abstract. It should accompany the Master's Final
Examination Request form, and be submitted three weeks before the date
of your Final Exam. This form is for the Thesis option.

Prospectus Information
When you are ready to go to prospectus, you will put your abstract on
this form. Once completed, you will turn this form in, along with the
Thesis Prospectus Scheduling Request form three weeks before the date
of your Prospectus.

Prospectus Scheduling Request
This form needs to be completed to schedule your Prospectus. The Thesis
Prospectus Information form should accompany the scheduling request. It
should be submitted three weeks before the date of your Prospectus.

Prospectus Examination Results
This form needs to be brought with you to the Prospectus Exam.

for Doctoral Students
Complete this form and submit it along with the Final Examination Information
and Final Examination Request forms three weeks prior to your scheduled
defense.
Dissertation
Proposal Information
Complete this form three weeks prior to your Prospectus exam, turn in
along with the Dissertation Prospectus Examination Request form.
Dissertation
Prospectus Examination Request
This form is needed to schedule your Prospectus Exam. Turn this form in,
along with the Dissertation Proposal Information form three weeks prior
to the Prospectus Exam.
Dissertation
Prospectus Examination Results
Bring this form with you to your Prospectus Exam.
Final
Examination Information
This is for your dissertation abstract. Turn this form in when you submit
the Final Examination Request form at least three weeks prior to your
defense.
Final
Examination Request
This form schedules your dissertation request. Like most other forms this
needs to be submitted three weeks prior to the defense.
Plan
of Study
Before completing 12-15 credit hours in the program work with your advisor
to develop your plan of study. Any changes to your plan of study need
to be documented using the Program Changes Request form.
Preliminary
Examination Request
This form must be completed at least three weeks before the date of the
scheduled Oral Defense of the Prelims.
Qualifying
Exam Results
Bring this form with you to the Qualifying exam.
